First criterion
Duplex ultrasound examines not only the shape of the vein but the direction of flow, valve function and where reflux begins.
Confirming the site and extent of reflux in the main veins — the great saphenous, small saphenous and perforating veins — is the important part.
Reflux on ultrasound does not by itself mean surgery or a procedure is decided.
The location and diameter of the refluxing vein, the severity of symptoms and any skin change are considered together in setting the direction of treatment.

Second criterion
If heaviness, swelling, pain, heat, numbness or night cramps keep returning and affect daily life, the state of the veins should be checked.
Spinal conditions, peripheral nerve problems and lymphatic circulation can cause similar symptoms, so duplex ultrasound is needed to see how far venous reflux is involved.

Third criterion
When pigmentation around the ankle, itching, eczema, lipodermatosclerosis, venous ulcers or superficial thrombophlebitis are present, a combined approach to treatment is needed

When symptoms are mild or there is no clear reflux, conservative care comes first

No. Even when no veins are visible, recurring heaviness, swelling, night cramps or numbness can point to a circulation problem. Only duplex ultrasound can give an accurate diagnosis.
Not every case calls for surgery. Depending on the symptoms and the ultrasound findings, it can be managed with compression stockings, oral medication or sclerotherapy. The direction is decided after a full examination.
Yes, these are common early symptoms. If they worsen in the afternoon or after long periods of standing, a circulation problem is possible. If they keep returning, an examination is recommended.
